Job summary

MHMR of Tarrant County is seeking a Direct Care Clinical Therapist/Social Worker/Case Worker to provide ongoing services for patients experiencing housing crises or homelessness within our MHMR Tarrant supportive housing program.

The navigator will assist patients with housing options, coordinate CAS referrals, and support access to housing through the referral and lease process, combining compassionate, person-centered care with crisis intervention skills.

Qualifications

• Bachelor's degree in health and human services field required.
• Bilingual skill and crisis service experience preferred.
• Eligible to obtain QMHP-CS certification; driver’s license required.

Responsibilities

• Provide ongoing services to patients in housing crisis or homelessness.
• Assist patients with housing options and coordinate referrals.
• Navigate CAS housing referrals through the housing referral and lease process.
