Job summary

Riverside University Health System – Behavioral Health is seeking a Social Services Assistant for the HHOPE program in Indio. You will support individuals experiencing homelessness, track housing placements, and coordinate with PSC, BHS III, and Housing Navigation teams.

Primary duties include data entry, referrals, and basic program education to members. The role requires strong organizational and communication skills, with attention to documentation in EPIC/HMIS and collaboration with housing

Qualifications

• Experience coordinating services for vulnerable populations preferred.
• Ability to communicate with diverse groups and document clearly.
• Basic knowledge of housing services and public assistance concepts.

Responsibilities

• Visit housing sites to verify placements and monitor progress.
• Assist with housing plans and documentation; coordinate referrals.
• Complete program forms and help members access services.
• Provide basic program education on expectations and resources.
• Coordinate with housing vendors and internal teams; share updates.
• Document interactions and updates in EPIC/HMIS.
