Job summary

Catholic Charities, Diocese of Trenton is seeking a Specialist to advocate for adults with mental illness and chronic/serious medical conditions. The position is based at IBHS Mercer – Steinert Ave, Hamilton, NJ, and requires coordinating services within the MESH Team alongside direct service provision.

Qualifications

Responsibilities

• Coordinate medical, mental health, social, and educational services for each consumer.
• Use EBPs to provide supportive counseling and recovery planning.
• Provide crisis intervention for consumers.
• Help consumers build positive interactions with family, neighbors, and landlords.
• Help consumers access community resources.
• Assist with medical/mental health needs, transportation, and care coordination.
• Discuss recovery goals with consumer and team; document plan timely.
• Track progress or regression and document monthly.
• Aid budgeting and financial management.

JOB SUMMARY

The Specialist is the primary advocate for adults with mentalillness & chronic/serious medical diagnoses. These consumers will be referred by the Division of Mental Health Services & from within our own contiuum of housing services to establish independent living situations which provide specialized supports. The Specialist must coordinate each individual's services with the rest of the MESH Team & participate in the direct provision of these services.

DUTIES & RESPONSIBILITIES

• Responsible for ensuring that all needed medical, mental health, social services, vocational/educational services are obtained & coordinated for each consumer.
• Using Evidence Based Practices ( illness management & recovery, motivational interviewing, & WRAP) provide supportive counseling which assists each consumer in successfully living in community settings.
• Provide Crisis Intervention for consumers.
• Assist consumers in establishing positive interactions with family, neighbors, landlord, etc.
• Assist consumers in becoming familiar with their community & the resources available to them.
• Assist consumers with both medical & mental health needs, providing transportation,as needed, & providing a communication link between health providers & the consumers.
• As a member of the MESH team , the Specialist discusses with the consumer and other Team members what Recovery goals the consumer wants to work on & how to proceed with that goal. The Specialist is responsible for documenting this plan & putting it in the clinical record, in accordance with regulatory time frames.
• The Specialist is responsible for documenting progress &/or regression in the attainment of goals & what interventions are planned. This documentation is done on a monthly basis.
• Assist the consumers with budgeting & financial management.

Position Requirements
