This position is responsible for helping to identify, secure, and maintain community-based housing for MLTSS/SNP/Medicaid Members and for developing, articulating, and implementing a broader housing strategy within the Contractor to expand housing availability/options. The Housing Specialist(s) shall act as the Contractor's central housing expert(s)/resource(s), providing education and assistance to relevant staff regarding supportive housing services for MLTSS Members.

Responsibilities:
  1. Develop a housing strategy for Horizon membership (specifically MLTSS/SNP/Medicaid).
  2. Identify and manage resources for housing options.
  3. Provide education to staff, Care Management, Member Advocates, and Utilization Management on housing options and resources.
  4. Assist with locating and placing members in appropriate housing, facilitating transitions to community settings.
  5. Complete reports to meet state requirements, including progress towards goals and quality monitoring.
  6. Act as a liaison with DMAHS staff for training and capacity building.
  7. Work closely with public and private housing resources and stakeholders, including HUD, DCA, NJ HMFA, public housing authorities, realtors, and online housing locator resources.
Education/Experience:
  • High School Diploma/GED required.
  • At least three (3) years full-time experience assisting vulnerable populations to secure accessible, affordable housing.
  • At least two (2) years work experience in a social or community service field, preferably with a public agency.
Knowledge:
  • Familiarity with public and private housing resources and stakeholders.
  • Knowledge of Medicare/Medicaid and MLTSS.
Skills and Abilities:
  • Strong interpersonal, advocating, verbal, and written communication skills.
  • Proficiency with Microsoft Office applications.
  • Good problem-solving, investigation, mediation, and conflict resolution skills.
  • Organizational skills and ability to work independently.
  • Sound judgment and professional demeanor.
Travel:
  • May require travel within New Jersey.
  • Must have a car with valid NJ license and insurance.

Note: Horizon BCBSNJ employees must reside in NJ, NY, PA, CT, or DE.

Salary Range: $69,500 - $93,030

This range considers education, experience, licensure, certifications, location, and internal factors. Horizon offers a comprehensive benefits package including health, retirement, PTO, incentive plans, wellness programs, volunteer time, and tuition reimbursement.
