We are seeking a Housing Specialist to join our team in our Shelocta, PA location supporting Indiana and Armstrong County.
Starting rate of $21.14/hour + mileage reimbursement
Position Details
The Housing Specialist within the ACT team is instrumental in meeting the housing requirements of individuals with co-occurring disorders. Working closely with Psychiatrists and Primary Care Physicians, the Housing Specialist aids in treatment planning by evaluating the influence of addiction on mental and physical health and aiding in decision-making processes. Responsibilities encompass advising on medication prescriptions for substance abuse disorders, evaluating physical health, and orchestrating medical, psychiatric, and substance abuse treatments. By ensuring consumers have stable housing, the Housing Specialist significantly bolsters the efficacy of the ACT program, ultimately promoting overall recovery and well-being.
The Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.
Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.
