Job summary

Wilder is seeking a dedicated Case Manager to provide culturally responsive, trauma-informed services and referrals to help families secure and maintain housing in Saint Paul, MN. The role is in-office with potential to transition to hybrid after completing training.

You will complete daily documentation, interpret provider assessments, and maintain accurate client records, with in-person home visits required. A strong benefits package accompanies the position.

Qualifications

• AA degree in Social Work, Psychology, Sociology, Human Services, or related field is required, with at least two years of relevant work experience.
• BA degree in a related Human Services field preferred, with at least one year of relevant work experience; strong substitute for the BA.
• Valid Minnesota driver’s license and proof of insurance preferred.
• This job requires Case Managers to conduct in-person home visits.

Job description

Family Supportive Housing is committed to creating, designing, and implementing community services from a proactive, holistic, and person-centered approach. As a valued team member, you will provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to maintaining or stabilizing housing. Complete daily documentation, interpret reports and assessments from providers, and maintain complete and accurate client records.

This is an in-office position with potential to transition to hybrid after demonstrating competency and completing training.

Qualifications:

AA degree in Social Work, Psychology, Sociology, Human Services, or related field is required, with at least two years of relevant work experience. BA degree in a related Human Services field preferred, with at least one year of relevant work experience. Strongly relevant work experience of at least four years may substitute for the BA degree.

A solid understanding of homelessness, mental health issues, substance abuse, navigation of housing resources (including eviction prevention), domestic violence, and community resources is preferred. Candidates must possess a valid Minnesota driver’s license and proof of insurance.

This job requires Case Managers to conduct in-person home visits.
