Job summary

North Suffolk is seeking a Housing Specialist to provide direct care in both site-based and mobile Emergency Respite settings. The role involves helping clients with daily living skills, transportation, money management, medication administration, and risk mitigation according to treatment plans.

Schedule: Tuesday - Friday 2pm-10pm; Saturday 8am-4pm. You will document progress, coordinate housing resources, and assist clients in setting immediate and long-term housing goals with the team.

Qualifications

• Education: High School Diploma/GED required; Bachelor's degree preferred.
• Experience: At least one year in case management or housing coordination.
• Skills: Driver’s license; MAP and CPR/First Aid certification within five months of hire.

Responsibilities

• Develop housing search expertise and assess client housing options.
• Liaise with staff, family, advocates, and housing staff/programs as needed.
• Document clients' progress in confidential records.
• Maintain housing resource board with current information on options and eligibility.
• Meet with clients upon admission and assess housing needs and goals; coordinate tasks.
