Job summary

The Housing Navigation Case Manager (HNCM) at Pine Street Inn will work within a team of case managers to develop and execute sustainable housing plans for currently homeless guests. The role focuses on reducing barriers to housing and creating individual housing plans that rapidly lead to safe, affordable housing with the necessary supports.

Attention to detail is essential as staff enter guest information into HMIS and coordinate with other PSI departments to deliver integrated services

Qualifications

Minimum of two (2) years delivering services to a challenged population often struggling with addiction and untreated or under-treated mental health concerns
Strong computer and math skills - including familiarity with MS Word and Excel
Practical experience in working collaboratively with homeless individuals creating housing plans and individual service plans
Experience accessing housing opportunities and subsidy programs for homeless individuals
Knowledge of the issues related to homelessness and housing

Responsibilities

Develop and execute sustainable and expeditious housing plans for currently homeless guests.
Engage with homeless guests to reduce barriers to housing and develop individual housing plans.
Enter guest information into HMIS with attention to detail and accuracy.
Provide exceptional customer service and coordinate with other PSI departments for integrated service delivery.
