Job summary

UCSF Health is seeking a Social Work Associate to support the Margot Mission Team in San Francisco. The SWA will provide individual treatment, case management, crisis intervention, and linkage to outpatient services for formerly homeless individuals with mental illness and chronic health issues.

Responsibilities

• Provide ongoing mental health services including individual, group, and family psychotherapy, crisis intervention, and case management for a caseload of formerly homeless clients at 1321 Mission St.
• Help clients stabilize, maintain housing, and link to long‑term mental health, substance abuse, medical services, and other programs; develop discharge plans.
• Conduct intake, assessment, service planning, coordination with DISH staff and 1321 Mission Team; advocate with agencies, families, and clients; provide crisis intervention as needed.
• Track client contacts, assessments, referrals, and eviction information; utilize Medi‑Cal billing; maintain client records per city/Med‑Cal requirements; document in charts.
• Meet program productivity standards—averaging 80 hours per month; assist in quality improvement; complete service sheets promptly.
