• Collaborate on trainings for Case Management Teams and partner agencies.
• Maintain a database of resources and programs to improve housing stability.
• Support housing navigation and assist with referrals and warm hand-offs.
• Conduct home visits to ensure tenant wellbeing and housing stability.
• Coordinate regular meetings with partners to streamline care.
• Assist in coordinating housing transitions and client move-ins/outs.
