Job summary

The incumbent will assess clients, monitor health and medication, provide crisis management, and deliver individual counseling in line with policy. Responsibilities include documenting services within 48 hours, coordinating care with the team, assisting with independent living, landlord communications, and transportation as required.

Qualifications

• Bachelor's degree required; Human Services field preferred.
• Minimum one year experience with severely mentally ill; housing/tenant experience preferred.
• Knowledge of counseling techniques and mental illness.
• Valid Florida driver's license, auto insurance and personal vehicle required.
• Must pass Level 2 background check and drug screen.

Responsibilities

• Assesses and screens consumers; monitors mental health status and medication compliance.
• Provides crisis management and individual counseling as needed.
• Documents services within 48 hours and maintains productivity standards.
• Supports independent living skills and tenant-related tasks; documents reports.
• Coordinates with team and other service providers to ensure appropriate care.
• Performs monthly apartment inspections and processes referrals as needed.
