Job summary

Community Bridges, Inc. (CBI) seeks a Client Care Coordinator to work at shelter sites in Mesa, AZ. You will coordinate housing and support services, linking individuals and families to medical, behavioral health, and social resources to help end homelessness.

The role involves data entry and end-of-shift reporting within CBI’s care continuum. You will collaborate with a multi-disciplinary team, follow applicable policies, and support clients through transition into stable housing and community

Qualifications

• Highschool diploma or GED required; Associate degree in Behavioral Health preferred.
• Minimum 12 months recovery from substance use and/or mental health disorders.
• Minimum 6 months paid related work experience in behavioral and medical crisis situations.
• 1-3 years full-time health care related work experience preferred.
• Valid identification required.
• Will complete CBI Peer Certification within 90 days of being hired.
• Arizona Fingerprint Clearance Card (Program Specific).

Responsibilities

• Coordinate care for individuals and families ending homelessness episodes.
• Coordinate care with RBHA and community-based services and connect to PCMH/PCP.
• Document interactions per AAC/ADHS- DBHS policies and program manual.
• Complete data entry to support program outputs and outcomes.
• Prepare end-of-shift reports detailing care coordination for each individual.
