Job summary

Sheppard Pratt is hiring an Entitlements and Housing Specialist for Chesapeake Connections. The role supports members in obtaining entitlements, developing budgets, and securing suitable housing aligned with each member’s level of care.

Responsibilities include coordinating with DSS and Social Security, assisting with housing inspections, and building relationships with landlords. Strong organizational skills and knowledge of mental health services are essential.

Qualifications

• High school diploma and 2+ years of related work experience; bachelor’s degree preferred in social work, psychology, counseling, or related field.
• Valid driver’s license with 3 points or less and access to an insured vehicle.
• Knowledge and understanding of severe and persistent mental illness.
• Proficient computer skills; working knowledge of Microsoft Word and Excel.

Responsibilities

• Assist members in accessing entitlements and benefits, initiating applications, gathering records, and tracking status.
• Coordinate with DSS, Social Security, and other agencies to secure entitlements.
• Coordinate medical insurance, food stamps, and Section 8 needs with members.
• Meet with members and staff to resolve money issues and manage personal budgets.
• Identify assisted living options, screen for suitability, and maintain current licensures and facility lists.
• Establish relationships with landlords and rental agents for housing opportunities for members.
• Serve as a liaison to treatment teams on housing issues and support housing acclimation for members.
• Provide transportation to members using a personal or agency vehicle.
