Job summary

Sheppard Pratt in Baltimore, MD invites applications for a Housing & Entitlements Specialist, Community Treatment. This full-time, on-site position pays $21.37 per hour and centers on helping members access entitlements and secure safe, affordable housing within appropriate care.

Responsibilities include coordinating benefits (insurance, food stamps, Section 8), developing personalized budgets, networking with DSS and other agencies, assisting with housing inspections, and guiding members

Qualifications

• A high school diploma and two years of related work experience.
• Bachelor's degree in social work, psychology, counseling, or related field preferred.
• Valid driver's license with 3 points or less and access to an insured vehicle.
• Knowledge and understanding of severe and persistent mental illness.
• Computer skills and knowledge of Microsoft Word and Excel.

Responsibilities

• Assists members in accessing entitlements, initiating applications, gathering records, monitoring status, and communicating results.
• Networks with DSS, Social Security, and other agencies to secure entitlements.
• Coordinates medical insurance, food stamps, and Section 8 needs.
• Develops, updates, and modifies personalized budgets for members.
• Identifies housing options, screens providers, and maintains licensure information.
• Serve as a liaison to treatment teams regarding housing issues and coordinate transportation.
