Pathways, Inc. in Bath, NY is hiring a Regular Full-Time Housing Case Manager to support individuals experiencing homelessness and coordinate housing solutions, employment readiness, and financial literacy while addressing barriers to housing.

The role requires an Associates degree with 3+ years of case management, strong communication, and a valid drivers license. Monday-Friday 8:30am-4:30pm with occasional late nights.

Qualifications

• Associates degree with 3+ years experience in case management.
• Experience working in homelessness, mental health and substance abuse.
• Strong communication skills.
• Strong sense of boundaries.
• Valid drivers license and ownership of a motor vehicle.

Responsibilities

• Provide support and services to individuals experiencing homelessness to obtain housing.
• Assist with employment readiness and financial literacy.
• Help clients meet program expectations and maintain housing.
• Identify and address barriers to housing and stability.
