Job summary

Wilder in Minnesota is seeking a Case Manager to deliver culturally responsive, trauma-informed case management and referrals to help families maintain stable housing. You will conduct in-person home visits, complete daily documentation, and maintain accurate client records.

This in-office role offers training with potential to transition to a hybrid arrangement after demonstrating competency. A driver’s license and solid background in social services are required.

Qualifications

• AA degree in Social Work, Psychology, Sociology, Human Services, or related field is required with two years of relevant work experience.
• BA degree in related Human Services field preferred with at least one year of relevant work experience; four years may substitute for BA.

Responsibilities

• Provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to housing stability.
• Complete daily documentation and maintain complete and accurate client records.
• Interpret reports and assessments from providers.
