Liberation Programs Inc. in Bridgeport, CT seeks a Homeless Outreach Specialist to support the FC CAN network in helping unsheltered individuals locate and sustain permanent housing.

You will conduct outreach, intake assessments, and housing planning, coordinating with landlords and service providers. The role emphasizes harm-reduction practices, data management, and ongoing client support for stabilization.

Qualifications

• 2 years’ experience working with homeless population and/or lived expertise.
• Experience with harm reduction approaches and delivering harm reduction services.
• Excellent written and verbal communication skills.
• Treats participants with respect and dignity, providing supportive services.
• Creative problem solver with strong organizational skills.

Responsabilités

• Provide street outreach through canvassing with FC CAN team outside normal hours.
• Conduct CAN intake and identify emergency shelter opportunities and housing plans.
• Collect IDs, documentation, income and disability verification; upload to HMIS.
• Add clients to the By Name List and develop Housing Services and Sustainability Plan.
• Maintain HMIS data entry and organize files; use Smart Sheet and Ddap.
• Assist clients in obtaining income via workforce referrals or SOAR connection.
• Provide ongoing case management and link to benefits and health services.
• Participate in local CAN housing solution meetings to advocate for housing opportunities.
• Coordinate housing search, landlord recruitment, and relocation assistance.
• Assist with lease signing, tenant rights and lease compliance.
• Complete DMHAS Acuity and Housing Sustainability plan and biennial assessments.
• Conduct regular home visits to promote long-term housing stability.
