Liberation Programs in Bridgeport, CT is seeking a Homeless Outreach Specialist to support the FC CAN network. The role focuses on street outreach, intake, housing placement, and ongoing tenancy support to help unsheltered individuals obtain stable, long-term housing.

The position requires hands-on outreach, documentation in HMIS, and collaboration with housing partners. Compensation is $45,000 to $54,000 annually, with a full-time schedule in Bridgeport.

Qualifications

• 2 years' experience with homeless populations.
• Experience with harm reduction and providing safer-use supplies.
• Excellent written and verbal communication.
• Ability to work independently and with diverse partners.
• Valid driver’s license and access to a vehicle.

Responsabilités

• Provide street outreach through canvassing with evening hours with FC CAN.
• Conduct CAN intake and identify emergency shelter opportunities.
• Collect IDs, income verification, homelessness verification, and disability verification and upload into HMIS.
• Maintain HMIS documentation and By Name List (BNL).
• Coordinate housing search, landlord outreach and relocation assistance.
• Identify housing options through ongoing outreach and cultivate landlord relationships.
• Complete DMHAS Acuity, Housing and Sustainability plan and biennial assessments.
• Conduct regular home visits for long-term housing stability.
