Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to conduct in-home health assessments for Medicare Advantage and other plan members. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible schedules (20–40 hours/week), no on-call, no weekends, and a sign-on bonus. Experience with EMRs and home visits is valued.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license in practicing state.
• Driver's license and access to reliable transportation for home visits.
• Ability to adapt to varied home environments and perform assessments.
• Eligibility timing: obtain licensing board approval if required by state laws.

Responsibilities

• Conduct comprehensive in-home health assessments (history, meds, vitals, exam).
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to close care gaps.
• Identify urgent or emergent situations and intervene appropriately.
• Educate patients on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
