Job summary

Momentum in Thousand Oaks, CA is seeking a House Coordinator to manage day-to-day operations of a group home under the Administrator/QIDP and nursing staff. You will coordinate DSPs, train staff, orient new residents, and supervise meals and activities to ensure quality care.

The role requires 18+ months of direct support experience, a high school diploma, a valid California driver’s license, and strong English communication. A commitment to safety and teamwork is essential.

Qualifications

  • Must be at least 18 years of age.
  • High school diploma or equivalent; required.
  • Valid and current proof of legal right to drive in California; required.
  • At least 18 months of Direct Support experience, working with individuals with disabilities under supervision.
  • Positive attitude with flexibility to meet the needs of the residential home.
  • Read, write, comprehend, and communicate in English well enough to perform essential duties.
  • Communicate and work professionally and cooperatively with others.
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam.
  • Able to run, push, pull, bend, squat, kneel, and lift up to fifty pounds.

Responsibilities

  • Assists Administrator/QIDP with instructing staff on implementing ISP objectives.
  • Coordinates Direct Support Professionals in cleaning and laundry duties.
  • Trains new staff regarding persons supported needs and housekeeping duties.
  • Provides new supported persons with an orientation to the facility and staff.
  • Assigns and monitors meal preparation, cooking, and cleaning; ensures menus are posted and followed.
  • Drives persons supported to appointments using company vehicle and assists on outings.
  • Provides direct support, as needed.
  • Performs other duties as assigned to meet department and/or business needs.

2:30 PM to 11:00 PM, five days per week, including weekends and holidays (full-time/40 hours per week)

House Coordinator Essential Responsibilities:
  • Assists Administrator/ QIDP with instructing staff on how to carry out active treatments and training objectives identified in each persons Individualized Service Plan (ISP)
  • Coordinates Direct Support Professionals (DSPs) in cleaning and laundry duties
  • Trains new staff regarding persons supported needs, individual training objectives, and housekeeping duties
  • Provides new supported persons with an orientation to the facility with introduction to staff and peers
  • In conjunction with the Dietitian and Administrator, assigns and monitors meal preparation, cooking, and cleaning; ensure menus are posted and followed; and prepare meals, as needed
  • Drives persons supported to appointments using company vehicle and assist supported persons on outings
  • Provides direct support, as needed
  • Performs other duties as assigned to meet department and/or business needs
House Coordinator Qualifications:
  • Must be at least 18 years of age
  • High school diploma or equivalent; required
  • Valid and current proof of legal right to drive in California; required
  • At least 18 months of Direct Support experience, working with individuals with disabilities under the supervision of a Qualified Intellectual Disabilities Professional (QIDP)
  • Positive attitude with flexibility to meet the needs and demands of the residential home
  • Read, write, comprehend, and communicate in English well enough to perform essential duties and responsibilities
  • Communicate and work professionally and cooperatively with other
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
  • Able to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
