Host Home Provider – Pueblo County & Surrounding Areas

Salary Range: $18,000.00 to $65,000.00/year depending on the clients’ disability level

HOST HOME PROVIDER POSITION

We seek experienced Host Home Providers willing to open their homes and foster individuals who cannot live independently or prefer not to live in communal settings. Providers share their homes and lives, offering a private bedroom, hands-on support, and ongoing daily care. You can assist individuals with medical concerns or behavioral challenges and make a meaningful difference as part of your family. Bonus Incentives:

1. Receive $100 upon application submission.

2. After the individual has been placed and remains in the home for 90+ days, receive an additional $400.

HHP Provider Duties:

• Assist with hygiene, meals, dressing
• Provide recreational activities to promote social development
• Include the client in family activities
• Accompany clients to outings like supermarkets, museums, parks, dances, and recreation centers
• Provide transportation as needed
• Assist with medical appointments and medication administration
• Document progress and medical records
• Train on life skills

Candidate Requirements:

• SOMB training or willingness to be trained
• Follow the Client's Service Plan restrictions
• No children in the home
• No internet access for the client at home
• High School diploma or equivalent
• Criminal and motor vehicle background checks
• Complete and maintain training in Medication Administration, CPR/First Aid
• At least 1 year of experience as a host home provider
• Experience with behavioral or medical needs preferred (e.g., CNAs, LPNs, RNs)
