Salary Range: $18,000.00 to $65,000.00/year depending on the clients’ disability level

HOST HOME PROVIDER POSITION

We are seeking experienced Host Home Providers who are willing to open their home and foster individuals who can no longer live independently in their community and prefer not to live in a communal setting. Providers share their homes and lives with individuals with Developmental Disabilities. As a Host Home Provider, you take the client into your home and provide them with a private bedroom, hands-on support, and ongoing daily care. You can help someone with significant medical concerns or behavioral challenges and make a difference in their life as they become part of your family. A bonus is received whenthese steps are completed:

1. The applicant will receive $100.

2. When the person receiving services has been placed and stays in the home for 90+ days, the remaining $400 will be received.

HHP Provider Duties and Responsibilities:

• Assist with hygiene care, meal preparation, dressing
• Provide a variety of recreational activities to enhance social development
• Include the client in family life and activities
• Accompany clients to supermarkets, museums, parks, dances, recreation centers, etc.
• Provide transportation as needed
• Assist with medical care, including appointments and administering medications
• Provide documentation of progress notes and medical records
• Provide training on life-learning skills

To be successful, candidates should:

• Have SOMB training or be willing to be trained
• Follow restrictions laid out in the Client's Service Plan
• No children in the home
• No internet access to the client in the home
• Have a High School diploma or equivalent (Required)
• Complete and pass criminal and motor vehicle background checks
• Complete training for Medication Administration, CPR/First Aid, and maintain ongoing training requirements
• Have at least 1 year of experience as a host home provider
• Experience with behavioral needs is preferred
• Experience with medical needs is preferred (e.g., CNAs, LPNs, RNs, etc.)
