POSITION

Support, Inc. is looking for an experienced Host Home Provider to contract with our company in Lakewood, Colorado. If your home is wheelchair accessible, you have the opportunity to open your own home to provide for someone with developmental disabilities. You will be contracting with Support Inc. to help improve someone's quality of life, especially those with significant medical concerns or behavioral challenges. SPECIFIC CLIENT INFORMATION/NEED:

• Client needs a fully wheelchair accessible home, preferably with a walk-in/roll-in shower or a large bathroom for lift access.
• Client will require assistance with transfers, bathing, toileting, and meal preparation.
• Client will have unsupervised time at home during the day and works one day a week in Lakewood.
• Client desires to maintain independence and stay connected with family for occasional weekend respite stays.

Provider Duties and Responsibilities

• Assist with hygiene care, meal preparation, dressing
• Provide recreational activities to enhance social development
• Include client in family life and activities
• Accompany client to various outings and provide transportation
• Assist with medical care, appointments, and medication administration
• Document progress notes and medical records
• Provide training on life learning skills

To be successful, candidates must:

• Have a high school diploma or equivalent
• Pass criminal and motor vehicle background checks
• Complete training for Medication Administration, CPR/First Aid, and maintain ongoing training
• Have at least 1 year of experience as a host home provider
• Have a wheelchair accessible home
• Preferably have experience with behavioral and medical needs

Support Inc. Provides:

• Paid initial and ongoing training
• Designated Support Team
• Assistance in finding Respite and home care Aids
• Ongoing support and assistance
• Client programming and events

Support Inc. is a private community services agency.

Job Type: Contract

Salary Range: $18,000.00 to $65,000.00/year depending on the clients' disability level
