Position Overview

Support Inc. is a highly regarded service agency serving individuals with intellectual and developmental disabilities. We are seeking an experienced Host Home Provider in Lakewood, CO, with a wheelchair accessible home to provide in-home services to adults with developmental disabilities.

Client Needs
  • Fully wheelchair accessible home, preferably with a walk-in/roll-in shower or a large enough bathroom for lift access.
  • Assistance with transfers, bathing, toileting, and meal preparation.
  • Unsupervised time during the day; client works one day a week in Lakewood.
  • Desire to maintain independence and connect with family, who may offer occasional respite stays.
Provider Responsibilities
  • Provide personal and hygiene care, assist with meals and dressing.
  • Engage in recreational activities to promote social development.
  • Include client in family activities and outings.
  • Transport client to various community locations.
  • Assist with medical appointments and medication management.
  • Document progress and medical records.
  • Train on life skills development.
Candidate Requirements
  • High school diploma or equivalent.
  • Pass criminal and motor vehicle background checks.
  • Complete training in Medication Administration, CPR/First Aid, and maintain ongoing training.
  • At least 1 year of experience as a host home provider.
  • Home must be wheelchair accessible.
  • Experience with behavioral and medical needs preferred (e.g., CNAs, LPNs, RNs).
