HOST HOME PROVIDER POSITION

We seek experienced Host Home Providers willing to open their homes to individuals unable to live independently in their community and who prefer a home setting over communal living. Providers share their home and lives, offering a private bedroom, hands-on support, and ongoing daily care. This role involves supporting individuals with medical or behavioral challenges, helping them flourish and become part of your family. HHP Provider Duties and Responsibilities:

• Assist with personal care, hygiene, meal prep, dressing
• Provide employment coaching to help clients find jobs
• Organize recreational activities to promote social development
• Include clients in family activities and outings
• Accompany clients to stores, parks, museums, dances, recreation centers, etc.
• Provide transportation as needed
• Assist with medical appointments and medication administration
• Maintain progress notes and medical records
• Offer life skills training

To be successful, candidates should:

• Have a high school diploma or equivalent (Required)
• Pass criminal and motor vehicle background checks
• Complete training in Medication Administration, CPR/First Aid, and maintain ongoing training
• Have at least 1 year experience as a host home provider
• Preferably experienced with behavioral needs
• Preferably experienced with medical needs (e.g., CNAs, LPNs, RNs)
