Overview

A Host Home provider is an independent contractor who opens their home and heart to an adult with a developmental disability. Supports may include assistance with personal care, hygiene, meal planning, medications, recreation, leisure activities, transportation, shopping, and more.

Responsibilities

• Provide daily care, supervision, and support in accordance with laws and the Individual’s Service Plan.
• Prevent and report acts of abuse, neglect, or exploitation.
• Document progress and incidents, and submit reports as required.
• Arrange and facilitate medical and dental care.
• Transport the Individual to appointments and activities.
• Notify supervisors promptly in case of serious health issues or emergencies.
• Maintain confidentiality of records.
• Attend scheduled meetings and complete required documentation.
• Assist with training relief staff and ensure compliance with training requirements.
• Communicate updates regarding the Individual’s status.
• Review and sign medication administration records (MAR) and related documentation.
• Coordinate medical orders and follow-up on labs and treatments.
• Support the Individual’s health and safety needs, including adaptive equipment and health checks.
• Limit travel as needed and perform other duties as assigned.

Qualifications
