Overview

A Host Home provider is an independent contractor who opens their home and heart to an adult with a developmental disability. Supports may include assistance with personal care, hygiene, meal planning, cooking, medications, recreation, leisure activities, transportation, shopping, and more.

Responsibilities

• Provide daily care, activities, and supervision in accordance with laws and the Individual’s Service Plan, respecting their emotional, intellectual, physical, and spiritual needs.
• Prevent acts of disrespect, abuse, neglect, or exploitation, and report any such acts immediately.
• Document the Individual’s progress and incidents, and submit reports as required.
• Arrange and oversee medical and dental care in line with the Service Plan and the Individual’s needs.
• Transport the Individual to appointments and community activities as per the Service Plan.
• Notify supervisors immediately in case of serious illness, injury, or death, after emergency care is provided.
• Maintain confidentiality of all records and information.
• Complete daily notes, attend scheduled meetings, and communicate regularly with the company.
• Assist in training relief staff and ensure compliance with documentation and training requirements.
• Report changes in the Individual’s condition or environment to the supervisor.
• Follow medical orders, document medication administration, and coordinate healthcare needs.
• Assist with adaptive equipment and ensure physician orders are current and signed.
• Complete required health checks and follow-up appointments.
• Perform limited travel and other duties as assigned.

Qualifications

• High School Diploma or GED
• Training in CAP Core Competencies
• Valid driver’s license, reliable vehicle, and good driving record
• Strong communication skills (verbal and written)
• Comfortable assisting with hygiene and medical routines
• Organizational and interpersonal skills
