Job summary

Mount Nittany Health is seeking a Clinical Performance Nurse (CPN) to join the hospitalist team in State College, PA. The CPN will help drive quality and efficiency improvements across the acute care episode without direct patient care, engaging physicians, and care teams to meet performance targets.

Qualifications include a registered nurse degree with a BSN preferred and at least 3 years acute care experience.

Qualifications

• RN degree completion required; BSN preferred.
• Minimum 3 years acute care nursing experience.
• Experience in case management, utilization management, or quality improvement preferred.
• Experience in nursing leadership would be a plus.
• Experience with clinical quality databases is a plus.

Responsibilities

• Participate in development of site-specific workflows to meet program goals and targets.
• Collaborate with Medical Director and Practice Manager to implement processes and lead optimization.
• Provide timely status updates and action plans.
• Educate provider teams on clinical performance initiatives and metrics.
• Coordinate with hospital departments to ensure delivery of optimized workflows.
