Qualifications

• Experience supporting high‑risk individuals and trauma‑informed care.
• Experience engaging with diverse communities in public health contexts.
• Ability to document case notes and coordinate with partners.

Responsibilities

• Provide support and services for gunshot, stabbing, and aggravated assault victims at hospitals.
• Deliver direct case management and emergency support to victims and families.
• Offer crisis response to victims and families after shootings or homicides.
• Advocate for victims and coordinate referrals to community services.
• Develop relationships with partner organizations for warm referrals.
• Participate in trainings and workshops for the division.
• Document client services and assist with reporting.
• Identify safety concerns and create long‑term safety plans.
