Job summary

Paradise Valley Hospital, part of Prime Healthcare, seeks a Social Services II to provide emotional support, crisis intervention and counseling across pediatric to older adult patients. You will perform psychosocial assessments and aid in treatment planning, coordinating discharge with a multidisciplinary team.

Qualifications

• Master’s Degree in Social Work or related field is required.
• Master’s Degree in Marriage and Family Therapy or related field is required.
• Licensure or registration within state timelines as applicable.

Responsibilities

• Provide emotional support and crisis intervention for patients across age groups.
• Offer counseling and information/referral services to patients and families.
• Perform psychosocial assessments and assist in Treatment Plan development.
• Coordinate discharge planning as part of a multidisciplinary team.
• Maintain clinical documentation for each client during length of stay.
