With a Master’s in Social Work and a state license, you’ll apply critical thinking, effective communication, and education to support diverse patient populations.

Qualifications

• Master's degree in Social Work required.
• LCSW/LMSW, LPC, or Social Work license as required by state of practice Upon Hire (Preferred).
• CCM certification preferred; 3+ years in related healthcare field required/preferred.

Responsibilities

• Coordinates psychosocial activities with the Interdisciplinary Team and Physicians to provide comprehensive discharge planning.
• Utilizes critical thinking to develop and execute effective discharge planning.
• Conducts comprehensive biopsychosocial assessments of patients and family to provide timely and safe discharge planning.
• Serves as a patient advocate.
• Provides patient and family education on identified post-hospital needs.
• Collaborates with clinical staff in the development and execution of the plan of care, and achievement of goals.
• Provides education to patients/families and the healthcare team regarding cultural/religious beliefs, ethics, abuse, neglect and financial exploitation, and advance directives.
• Makes referrals to community resources as appropriate.
• Coordinates discharge and post-hospital care through community resources.
• Serves on hospital committees when requested.
