Qualifications

• Licensure as AAC or LSWAIC required.
• BLS, First Aid, Handle with Care, and Verbal De-escalation training provided.
• Preferred: LMHC, LICSW, LMFT, LASW, or Psychologist.
• Ability to conduct psychosocial assessments and coordinate discharge planning.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, behavioral, developmental, social, and support needs.
• Provide individual, family, and group interventions to support treatment goals.
• Develop and update individualized treatment plans with patients and families.
• Facilitate treatment planning meetings and coordinate care with the treatment team.
• Assist patients and families with identifying resources and discharge planning needs.
• Monitor progress, evaluate treatment effectiveness, and adjust interventions as needed.
• Facilitate family meetings and maintain clear communication with the team.
• Maintain timely, accurate, compliant clinical documentation.
• Ensure confidentiality and regulatory compliance.
