Qualifications

• Master’s Degree in social work (CA/HI licensure required)
• One year experience in an acute or long term medical setting (preferred)
• Two years' experience in an acute or long term medical setting (preferred)

Responsibilities

• Collaborates and facilitates post-discharge planning and transitions.
• Assists patients and families in determining appropriate hospice care level and refers to outpatient resources.
• Initiates a care plan and patient teaching; reassesses care plan based on physical and emotional needs.
• Develops a Plan of Care (POC) based on patient assessment including psychosocial factors.
• Performs other job-related duties as assigned.
