Astera Health is seeking a Licensed Social Worker to support discharge planning and connect patients and families with needed services. The role involves collaboration with physicians, nurses, and other clinic staff to ensure patient-centered care.

The position requires a Bachelor’s degree in human services or related fields and an active Social Worker license, with responsibilities including referrals to long-term and community-based services and ensuring patient self-determination in care

Qualifications

• Bachelor’s degree in human services or related field such as Social Work, Psychology, Human Services, Counseling, or Sociology.
• 2 years’ experience in human services related work in hospital, nursing home, or county human services agency.
• Active license as a Licensed Social Worker.

Responsibilities

• Supports the mission of Astera Health through personal conduct and behaviors that contribute ideas and suggestions, is helpful and friendly, demonstrates regular attendance and punctuality.
• Be a role model of Astera Health culture and core values.
• Serves as an integral part of the patient care team working with providers, nursing, and other clinic staff as well as the patient and their supports.
• Assists in planning and conduction of programs to assist patients with social problems and improve community health and counseling services.
• Assists in organization of support groups for patients and family members.
