Qualifications

• Master’s degree in Social Work (MSW) required.
• Licensure: LCSW preferred; applicable state certification preferred.
• Experience: At least 1 year of professional hospital/healthcare setting preferred.
• Experience with data management tools.

Responsibilities

• Completes psychosocial assessments for post-hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Collaborates with the Clinical Care Management team to assess and evaluate the appropriate level of care across the continuum.
• Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial aspects of illness and its effects on social support systems.
• Provides information and education to patients on community resources and post-hospital care options; makes referrals as needed.
• Reports suspected abuse/neglect/exploitation; liaises with Department of Social Services during evaluation/investigation.
• Responds to consultative requests after normal working hours.
