Hospital Sisters Health System (HSHS) seeks a part-time Social Worker in Green Bay, WI, in the Care Management department. The role involves coordinating social services for patients of all ages, supporting families, and providing short-term counseling. Schedule is PRN-on call with hybrid work arrangements at St.

Vincent's Hospital. QUALIFICATIONS include a Master’s in Social Work, WI CSW or IL LSW licensure, BLS, and relevant healthcare social work experience. Travel may be required.

Qualifications

• Master’s degree in social work is required.
• Bachelor’s degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master’s degree.
• Licensed as a CSW in Wisconsin or LSW in Illinois is required.
• Wisconsin CAPSW/CISW/LCSW or Illinois LCSW preferred.
• Eligible for temporary social worker license while completing licensure requirements.
• Basic Life Support (BLS) required.
• Travel, if required, may require a valid driver's license and transportation.

Responsabilités

• Coordinate and provide social services for patients of all ages in care settings.
• Help patients and families navigate the medical system and communicate needs to the care team.
• Assess and monitor mental and emotional health; provide short-term counseling as needed.
• Support discharge planning and care coordination across Illinois and Wisconsin.

Formation

Master’s Degree in social work

Bachelor’s degree in social work + 5 years social work experience

Licensed as CSW in Wisconsin or LSW in Illinois

CAPSW/CISW/LCSW preferred in Wisconsin; LCSW preferred in Illinois

Temporary social work license allowed during licensure period

Basic Life Support (BLS) certification

Driver’s license and transportation if travel is required
