Job summary

Mercy in Joplin, MO seeks a Care Manager to join the interdisciplinary team. The role assesses, plans, advocates, and coordinates care from admission to discharge, ensuring a safe transition post hospitalization.

As a PRN/As Needed position, you will contribute to performance metrics and best practices, support patient experience, safety, and quality of care, and collaborate with physicians, nurses, social workers, and discharge planners.

Qualifications

• Master's degree in Social Work (or MSW pending conferment)
• 0-2 years' experience in acute care hospital setting
• Current LMSW or LCSW license in the state of employment
• BLS (CPR) at hire or within 90 days; Certification in Case Management preferred
