Job summary

San Juan Regional Medical Center in Farmington, NM is seeking a Speech Language Pathologist to evaluate and treat patients, perform initial evaluations, discharge summaries, and re-evaluations, and provide daily treatment sessions in accordance with department policies.

Requirements include a current New Mexico SLP license with CFY or CCC, CPR/BLS certification, and preferred experience in Modified Barium Swallow studies. Strong communication and teamwork are essential.

Qualifications

• Current New Mexico SLP license, CFY or CCC.
• BLS/CPR certification.

Responsibilities

• Interpretation of referrals & orders to check for appropriateness, indications, and contraindications
• Initial evaluations
• Discharge summaries
• Re-evaluations
• Daily treatments: Are performed & documented in timely manner
• Education and instruction as indicated, including, but not limited to: patient, family, other staff, or follow up facilities
• Transportation of patients as needed
• Maintain, gather, clean, set up all equipment per facility policies
• Daily recordkeeping, including (but not limited to) evaluations, daily treatment notes, discharge notes and daily charges, and all communication/education provided (per facility / department policy)
• Each employee is responsible for implementing SJRMC’s Service Standards into their daily work: Safety, Courtesy, Effectiveness, and Stewardship
• Other duties as assigned
