Concord Hospital in New Hampshire seeks a pharmacist under the direction of the Director of Pharmacy to ensure safe and appropriate use of medications throughout the hospital, including interpreting orders, obtaining, preparing and dispensing drugs, and providing drug information. The role requires NH licensure, PharmD or B.Pharm, and desirable BCPS and residency experience.

Qualificações

• PharmD degree or Bachelor’s degree in Pharmacy from an accredited program.
• NH Pharmacist license or reciprocity pending.
• Board Certified Pharmacotherapy Specialist (BCPS) is desirable.
• A Pharmacy residency is desirable.

Responsabilidades

• Performs medication order entry and/or verification per policy.
• Coordinates daily pharmacy distribution.
• Represents and promotes pharmacy within and outside the department.
• Performs other duties as required or assigned.
• Identifies and resolves medication-related problems.
• Takes ownership of issues and follows to completion.
• Provides accurate drug information to staff, providers, and community.
• Assists physicians and residents with pharmacokinetic dosing and monitoring.
• Establishes a pharmacy practice specialty and acts as a resource.
• Documents and monitors all interventions and outcomes.
