Duties & Responsibilities:

• Prepare and dispense medications accurately and efficiently in accordance with physician orders and departmental protocols.
• Provide drug information and pharmacological guidance to the multidisciplinary healthcare team to support safe and effective patient care.
• Monitor patient drug therapy, assess clinical responses, and recommend adjustments based on patient age, renal/hepatic function, and pharmacokinetic principles.
• Perform patient education on medication use, including proper dosage, administration timing, potential side effects, and interactions with other prescribed or OTC medications.
• Ensure compliance with formulary guidelines and policies for both formulary and non-formulary medications.
• Maintain accurate documentation and adhere to all regulatory, safety, and institutional standards.

Skills & Qualifications:

• Strong knowledge of pharmacology, pharmacokinetics, and medication management.
• Ability to accurately dispense medications in a fast-paced healthcare environment.
• Skilled in assessing therapeutic appropriateness and providing evidence-based dosing recommendations.
• Excellent communication skills for educating patients and advising clinical teams.
• Detail-oriented with strong critical thinking and problem-solving abilities.
