TeamHealth is seeking an experienced advanced practice clinician for hospital medicine at HCA Florida Putnam Hospital in Palatka, FL. 7-on/7-off schedule with daytime shifts from 7am to 7pm and strong team support.

Qualifications

• Current Florida NP or PA license or eligibility to obtain licensure.
• National board certification as NP or PA.
• Experience in inpatient hospital medicine or related acute care areas.

Responsibilities

• Provide comprehensive care for adult hospitalized patients.
• Perform admissions, daily evaluations, and follow-up assessments.
• Develop treatment plans and manage acute and chronic conditions.
• Collaborate with physicians, nursing, specialists, and case management.
• Assist with care coordination and discharge planning.
• Maintain accurate documentation in the EMR.
• Communicate effectively with patients, families, and healthcare team.
