Qualifications

• Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
• License to practice pharmacy in the state of practice and dispense controlled substances
• Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
• Effective communication, people management, and project management skills.
• Immunization certification if applicable to practice site needs
• Prior hospital experience
• Hospital experience as an intern or pharmacy student
• Previous experience working in an integrated healthcare delivery system
• Board Certification
• Health System Residency

Responsibilities

• Reviews patients' medication orders for allergy interactions, duplication of therapy, and drug interactions; provides evidence-based interventions.
• Oversees medication preparation and delivery to care areas; ensures secure storage and workflow.
• Counsels patients and promotes medication education and adherence; provides professional service.
• Completes regulatory training and ensures compliance with laws, policies, and SOPs.
• Supervises interns, technicians, and support staff; provides clinical consultation to health professionals.
• Solves problems related to equipment, maintenance, drug availability, staffing, and patient care.
• Identifies opportunities to streamline processes and improve patient safety; participates in meetings with feedback.
• Supports clinic/hospital directives including clinical, compliance, and operations.
• Supports local and enterprise leadership initiatives.
