You will collaborate with admissions coordinators, RN Case Managers, and clinical teams to refine discharge plans and ensure smooth transitions from LTACH to post-hospital settings. Strong communication and problem-solving skills are essential.

Qualifikationen

• 2-year associate degree in a health-related field or bachelor’s (Social Work preferred) or clinical license (LPN, RN, RT, PT, OT, or similar)
• Experience with patient care and interaction, preferably in LTACH or acute care hospital setting
• Ability to develop and maintain positive working relationships

Aufgaben

• Provide discharge planning and coordination for a group of patients.
• Coordinate with patients, families, and referral/community resources to address discharge barriers (financial, clinical, psycho-social, logistical).
• Communicate with external agencies to provide referrals and ensure smooth transitions from LTACH to post-hospital settings.
• Collaborate with admissions coordinators, RN Case Managers, and clinical teams to refine discharge plans and options
