Job summary

Qualifications

Responsibilities

• Process physician orders based on the patient’s choice and based on insurance contract agreements with certified agencies.
• Coordinate all Home Health, Durable Medical Equipment, Orthotics and Infusion.
• Utilize electronic referral system to send referrals for all post discharge needs to licensed providers.
• Assure follow-up with companies for the coordination of benefits and services.
• Obtain any required forms for completion with the physician attending, qualified practitioner or ordering physician for completion.
• Assist the social worker with obtaining patient choice when warranted.
