Qualifications

• Current state Pharmacist license.
• B.S. in Pharmacy required; PharmD preferred.
• Completion of PGY-1 or >3 years of qualifying experience.
• PGY-2 training preferred.
• Board certification in a BPS clinical specialty preferred.
• BLS Required.
• ACLS Preferred.
• Experience in hospital pharmacy operations preferred.

Responsibilities

• Coordinate pharmacy department activities to deliver high-quality pharmaceutical care.
• Spend 50%+ of professional time in direct patient care.
• Conduct rounds and patient counseling with other health professionals.
• Initiate, modify, or discontinue patient-specific pharmacotherapy.
• Support a range of services including medicine, ICU, obstetrics, pediatrics, emergency care, oncology, perioperative, geriatrics and outpatient services.

Education

B.S. degree in Pharmacy

Doctor of Pharmacy degree

Completion of PGY-1 or >3 years of qualifying experience

Postgraduate clinical training PGY-2

Board certification in a BPS clinical specialty

BLS Certification

ACLS Certification preferred

Hospital pharmacy operations experience preferred
