Job summary

Mercyhealth is seeking a Case Management Social Worker to coordinate and facilitate patient care, discharge planning, and resource referrals. The CMSW will collaborate with the Patient Navigator and multidisciplinary team to improve outcomes while ensuring regulatory compliance.

Qualifications

• Bachelor's degree (BSW or equivalent) required.
• LSW or CSW licensure in state of practice required.
• Case Management Certification (ACMA) preferred.

Responsibilities

• Provides psychosocial assessment, intervention and refers to community resources for hospital patients in various care settings.
• Develops and implements psychosocial treatment plans including crisis intervention and bereavement services.
• Assesses patients for discharge planning needs, barriers, supports, and resources.
• Develops individualized, patient-focused plans for continuing care needs.
• Educates patients and families on resources and options available.
• Facilitates and assists with legal issues within scope (POAHC, Guardianship, Patient Rights, Adoption).
• Reviews advance directives and helps patients complete new documents.
• Communicates with physicians, patients, families and multidisciplinary team to ensure patient self-determination.
• Provides education and support regarding diagnosis, illness, treatment and life situations.
• Maintains knowledge of community resources to address patient needs and strengthen support networks.
• Documents plan of care and progress in the medical record in a timely manner.
• Facilitates crisis intervention services in hospital settings.
• Identifies and reports child and elder abuse to local authorities.
• Liaises with county officials for protective services, aging, disabilities, domestic violence, mental health, and economic support.
• Maintains knowledge of payer regulations impacting the plan of care.
• Negotiates care issues (indigent issues, resource availability).
• Promotes customer satisfaction and intervenes at critical moments of service.
• Monitors inpatient readmissions for potential discharge issues.
• Provides follow-up with patients as needed.
• Identifies avoidable day information for organizational tracking.
