Qualifications

• Minimum Education & Experience: Relevant Bachelor's degree; OR Current state clinical licensure; OR 4 years relevant experience required.
• Demonstrates critical thinking and problem-solving in patient care and discharge planning.
• Ability to establish proactive relationships with interdisciplinary team members and providers.

Responsibilities

• Develops a plan of care for assigned patients with physicians and the multidisciplinary team.
• Monitors care progress through assessments and records to ensure desired outcomes.
• Identifies and resolves barriers to patient care and actively participates in discharge planning.
• Documents medical necessity and coordination activities in the health care record; ensures timely updates for payors.
