Qualifications

• Bachelor's degree in case management or health care for RN track.
• Current Registered Nurse license in the state worked.
• Master's Degree in Social Work and LMSW/LCSW or be eligible to become licensed within 6 months of hire.
• BLS certification may be required for acute care.
• Typically 2 years of clinical experience with strong teamwork, communication and time management.

Responsibilities

• Conduct initial assessments on patients and participate in daily multidisciplinary rounds.
• Coordinate discharge planning and communicate the plan to patients, families, and the medical team.
• Delegate tasks to the transitional care associate.
• Establish collaborative relationships with physicians, payers, and other health care team members.
• Educate internal staff on case management concepts and assist with integration into daily practice.
• May supervise other staff.
• Understand Medicare/Medicaid benefits and community resources relevant to health care and discharge planning.
