As the Care Coordination Case Manager, you will provide comprehensive care coordination for patients as assigned. This position is accountable for the clinical quality of Care Coordination services delivered by both internal and external staff and identifies/resolves barriers that may hinder effective patient care. Case Managers cover units throughout the hospital, generally working in multiple areas each day. Responsibilities include conducting initial assessments, participating in daily multidisciplinary rounds, communicating the discharge plan and updates to patients, families, and the medical team, and delegating tasks to a transitional care associate.
Schedule: Typically Monday‑Friday 8:00 am–4:30 pm, or as determined by caseload.
This position provides comprehensive care coordination for patients as assigned. The intensity of care coordination is situational and appropriate based on patient need and payer requirements. The goal is to empower the patient and family to participate fully in the discharge planning process. The position provides developmentally appropriate care for the population served, including planning for safe discharge, continuity of care, and recognizing and planning for the unique needs of all ages and patients with physical, mental, or chronic illness.
Registered Nurse (RN): Must possess knowledge of case management or utilization review, typically obtained through completion of a bachelor’s degree in case management or healthcare. Requires current R.N. license in Arizona.
Social Worker: Requires a Master’s Degree in Social Work and a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW), or an MSW with the requirement to become licensed within six months of hire. States not recognizing an LMSW require a Master’s Degree and a licensed social worker.
For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.
Requires a proficiency level typically achieved with two years of clinical experience. Must demonstrate critical thinking skills, problem‑solving abilities, effective communication skills, and time‑management skills. Must work effectively in an interdisciplinary team format. May be required to take rotating call based on acute facility needs. For case management positions in acute facilities, a minimum of one year of case manager experience in an acute care hospital is required.
Certification for CCM (Certified Case Manager) preferred.
Additional related education and/or experience preferred.
