Job summary

Regions Hospital in Saint Paul, MN is seeking a Care Management Assistant to help ensure safe, timely transitions of care. You will partner with RN and Social Work Care Managers to coordinate referrals, post-acute resources, insurance authorizations, patient and family communication, documentation, and data collection.

You will collaborate with the interdisciplinary care team to identify and elevate barriers that could delay a patient’s transition from the hospital.

Qualifications

• Associate degree or equivalent education/experience in healthcare, human services, business, or related field.
• Previous experience in healthcare, patient-facing care, care coordination, or administrative support.
• No license or certification required.

Responsibilities

Join our Care Management team and help support safe, timely transitions of care for patients. In this role, you’ll partner with RN and Social Work Care Managers to coordinate referrals, post-acute and community resources, insurance authorizations, patient and family communication, documentation, and data collection. You’ll collaborate with the interdisciplinary care team to identify and elevate barriers that could delay a patient’s transition from the hospital.

Work Schedule:

FTE 1.0 (40 hrs. weekly), Day Shift, Monday-Friday, No holidays

Required Qualifications:

• Associate degree in healthcare, human services, business, or a related field, or an equivalent combination of education and relevant experience
• Previous experience in healthcare, patient-facing care, care coordination, or administrative support
• No license or certification required

Preferred Qualifications:

• Bachelor’s degree
• 2+ years of healthcare experience
• Experience in hospital care management, care coordination, discharge planning support, health unit coordination, medical assisting, patient care, or a related setting
• CNA, HUC, or other relevant healthcare training or certification
